Case Management
Our team of case managers works with youth to develop individualized plans that address short and long-term goals towards stability. This includes connections to available housing, applying for identification documents and/or health benefits, receiving parent and prenatal care, family reunification, and connections to drug rehabilitation, mental health services, and employment support.
Our staff has training in the following areas:

- Substance Abuse Counseling Certification
- Preparing SSI Disability Claims
- Domestic Violence counseling certification
- Care Transitions Intervention Coach Training
- Fair Housing/Housing Rights for People with Disabilities
- Permanent Supportive Housing Services
- Compassionate Response to Cluttering and Hoarding
- Homeless Management Information System
- Mental Health First Aid
- Housing First Model
- Intensive Case Management Services
